Qualified Social Worker - Ongoing Support Team

We are looking to recruit a Qualified Social Worker - Ongoing Support Team with the Social Care industry in Wiltshire County Council. The Qualified Social Worker - Ongoing Support Team vacancy in Wiltshire County Council will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.

Responsibilities include:

• Creating and carrying out reviews of the care packages


• To undertake comprehensive social care assessments and reports with detailed Support Plans as appropriate


• Working with families, carers and communities to help them make informed decisions, enabling them to clarify and express their needs and contribute to service planning


• Assessing / reassessing complex health and social care needs


• Requesting funding


• Completing Decision Support Tool (DST) to establish eligibility for health care


• Attending MDT meetings

Experience preferred:

• Post qualifying experience of working within an Ongoing Support within the UK statutory setting.

The Team:

• Well-structured organised team, growing their social worker staffing levels.

Minimum Requirements:

• HCPC Registration


• 1 year post qualifying experience within UK Social Care industry


• Enhanced DBS/CRB (we can obtain for you)


• Eligibility to work in the UK
